Use these forms to track your progress in our...**28 Days to better golf and a healthier lifesytle** program. Below you will find helpful forms to track your progress while performing various tasks throughout the week and duration of the course. At the end of each week you have the option of submitting your results to us for individual monitoring. Our physician, Dr, Smith, will review your results and contact you of any irregularities should they appear from your results. Our golf professional, Todd Miller is always interested in seeing progress. Please email your copies of these forms to info@gtagolflessons.com for feedback from our professional staff.

Using the "doc" (or middle file) will allow you to fill in results, save and email back to us. Also the zip file will do the same but you will have to unzip on your computer after you download.
